What a Checked Note Looks Like
One report per transcript-and-note pair, read in the order that matters. Contradicted and unsupported statements come first. It works on pasted text from Krasyn, Freed, Heidi, Nabla, Abridge, DAX, or any other scribe that produces text.
One label per statement
The note is split into its sentences. Each one is labelled Supported, Unsupported, Contradicted, Scaffolding, or Unverified against the transcript you pasted. Unsupported means not in the transcript, nothing more. An exam finding the clinician saw but never said aloud lands there too.
Three flags from plain code
A number in the note that does not occur in the transcript. A denial about a topic the transcript raised but never actually denied. Specific content where the transcript only has an inaudible marker. These come from rules, not a model, so they are identical on every run.
Omissions, listed separately
Clinically material facts the transcript stated and the note does not carry, each with the short excerpt that establishes it. A note can be faithful and still incomplete, so the two failures are reported apart.
Every report states whether the model judge ran, ran on part of the transcript, failed, or did not run. Statements it did not assess are labelled Unverified and never counted as Supported. Within your account the same transcript and note return the same stored report. We publish no accuracy figure for Note Check, because it has not been measured against a clinician-adjudicated reference set. You read the whole note.
The Scribe That Checks Its Own Work
If you want Krasyn to draft the note as well, the Scribe runs the same kind of check on its own drafts before you sign. You stay responsible for what you sign no matter who drafted it, so the draft shows you what it is based on and where it is weakest, and it tells you when the transcript check could not cover the whole visit.
Untraceable statements are removed
Statements in the draft are checked against the session transcript. What the check cannot trace back is taken out and listed under its section, so you can see what was dropped rather than wondering what was invented. When a visit runs long enough that one pass cannot cover all of it, the note says so instead of implying it was fully checked.
Confidence per section, not per note
Each section carries its own score and review guidance, and links back to the transcript spans it was drawn from. “Review the whole note” is advice nobody follows. Knowing that two of four sections need your attention is something you can act on.
The signed note remembers who wrote what
Per-section attribution of what the AI drafted versus what you changed stays with the signed note, alongside whether the transcript check ran fully, ran against part of the visit, or did not run at all. An unreviewed AI section blocks signing until you edit it or explicitly accept it.
Grounding, review and attribution are mechanisms. They do not make a draft correct, and they do not replace your clinical judgment. We publish no accuracy figure and no documentation-time figure, because we have not measured one.
