Note Check, the deterministic layer
The interactive panel on the Note Check page runs the deterministic layer of our report engine in your browser: it reads the note structure, sets scaffolding aside, and flags numbers the transcript never carried (num-001) and denials about topics that were raised and never denied (neg-001). The pasted text is not uploaded, stored, or sent anywhere. The model judge, per-statement verdicts, and the omissions list run only in the free account, so a clean run here is not a verdict on the note.

Select the highest tier met for problems, data, and risk, and the calculator applies the 2021 AMA rule that the level is set by the highest tier supported by at least two of the three columns. It also supports the total-time path, for new and established patients. It is a structured aid for applying the MDM guidelines to a visit you describe: it does not read your chart, and the code it suggests is not a billing determination or a payer-specific ruling.

It uses approximate CMS-HCC V24 coefficients and is built for understanding how the model behaves, not for submission. CMS is phasing in V28, which renumbers many categories, so confirm the model your plan year actually uses before relying on any figure.

It also carries an E/M level calculator, an HCC risk stratification calculator, and denial-appeal and prior-auth reference tools. Confirm anything billable against your own current code set.

Paste a visit transcript and the note any AI scribe drafted from it, whether Krasyn’s or another vendor’s. Note Check reports, statement by statement, what the transcript supports, what it does not, and what it contradicts, plus the facts the note left out. It never edits the note, it compares against the transcript you paste rather than the chart, and we publish no accuracy figure for it.
What it costs: A free Krasyn Scribe account, no card. Each check uses one AI-drafted note from the free allowance of 10 notes a month.
